Consultation & Personalized Planning
Your journey begins with a private one-on-one consultation with Dr. Schwartz at our Beverly Hills office (or via virtual consultation for those traveling from out of town). During this meeting, you’ll discuss your goals in detail – how do you envision your chest?
You can even bring reference photos or review our portfolio of past patient results, including other non-binary top surgeries, to help communicate your preferences.
Dr. Schwartz will perform a gentle exam to evaluate your chest skin, existing tissue, and anatomy. Using this information and your input, he will recommend a surgical plan tailored to you. This plan covers the surgical technique (type of incision, how much tissue to remove or whether to use implants, nipple resizing/placement, etc.) and expected results.
No two surgeries are alike – your plan is truly made for your body and vision.
During the consultation, we also review practical considerations to ensure you are ready for surgery. Dr. Schwartz follows widely accepted guidelines (such as WPATH standards) to prioritize your safety and well-being. Once a surgical plan and any prerequisites are in place, we’ll schedule your procedure. Surgery Day
On the day of your top surgery, you will arrive at our accredited surgical facility in the morning. You’ll meet with Dr. Schwartz again to review the plan and make any final markings on your chest while you’re upright (to guide incision placement or implant positioning).
Afterwards, you’ll be gently placed under anesthesia by our expert anesthesiologist. Dr. Schwartz utilizes his Schwartz Rapid Recuperation protocols from the very start. This includes using a refined form of general anesthesia called TIVA (Total Intravenous Anesthesia). TIVA is a gentler anesthesia method that often results in a quicker wake-up and less post-operative nausea compared to traditional anesthesia. Throughout the procedure, your comfort and safety are our top priorities.
The surgery itself will follow the customized plan we designed together.
After the surgical work is complete, Dr. Schwartz will close all incisions with fine sutures. In many cases, he uses techniques that do not require post-surgical drains, but if a drain is needed (more common in double-incision mastectomies), it will be a small tube placed temporarily to prevent fluid buildup and will be removed in a few days. Finally, your chest will be dressed with a specialized compression garment or bandages to support healing and minimize swelling.
You will wake up in our recovery area under the care of our attentive nursing staff. Most patients feel groggy but comfortable as the anesthesia wears off.
Thanks to the TIVA approach, you’ll likely feel clear-headed faster and with minimal nausea. Once you’re fully awake and stable (usually within an hour or two), you’ll be discharged to continue recovering in the comfort of your home or hotel.
We’ll review all your aftercare instructions with your support person, ensure you have any prescribed medications and schedule your first follow-up visit. You’ll leave the surgical center knowing exactly how to care for your chest and when to contact us with any questions or concerns.
Recovery & Follow-Up Care

In the first few days at home, you’ll need to get plenty of rest and avoid any strenuous activity. However, we encourage gentle movements as soon as you’re able: simple things like walking around your home for a few minutes at a time can boost circulation and help you feel better.

Over the next few months, swelling will continue to go down, and your incision lines will fade. We will provide tips for scar care (such as topical silicone or massage techniques) to help your scars heal as subtly as possible.
By about 3-6 months post-surgery, you’ll see the true results: a chest that looks and feels much closer to the way you always envisioned.
